Should Metformin Remain the First-Line Therapy for Treatment of Type 2 Diabetes?

Metformin has been associated with improvement in glycemic control, weight neutrality, and low cost as well as low risk for hypoglycemia. However, recent GLP-1 RA and SGLT-2 inhibitor outcomes trials have impacted the guidelines for diabetes management. The authors point out that metformin lacks robust cardiovascular outcomes data compared to newer glucose-lowering agents like GLP-1 receptor agonists and SGLT2 inhibitors, which have demonstrated additional cardiovascular and renal benefits in high-risk patients. The article suggests that while metformin remains a valuable initial therapy, treatment decisions should be individualized, considering patient-specific factors and the potential advantages of newer medications. Clinical Outcomes of a New Foot-Worn Non-Invasive Biomechanical Intervention Compared to Traditional Physical Therapy in Patients with Chronic Low Back Pain. A Randomized Clinical Trial

A randomized controlled trial published in the Global Spine Journal evaluated the effectiveness of a novel foot-worn, home-based biomechanical intervention (HBBI) compared to traditional physical therapy (TPT) in patients with chronic low back pain (CLBP). Results demonstrated that the HBBI group experienced a significant reduction in pain, with a 56% decrease on the Numeric Rating Scale (NRS), compared to a 26% reduction in the TPT group. Additionally, the HBBI group showed superior improvements in functional ability and quality of life, suggesting that this non-invasive, home-based intervention may serve as an effective adjunct to standard care for CLBP patients. Measles Math: What to Know About 1,001 Measles Cases Across The Country

As of May 9, 2025, the United States has reported 1,001 confirmed measles cases across 31 jurisdictions, marking the second-highest annual total in 25 years. The largest outbreak is centered in West Texas, with 709 cases primarily affecting undervaccinated communities. Three deaths have been reported: two unvaccinated children in Texas and one adult in New Mexico. Health experts emphasize that maintaining a 95% vaccination rate is crucial to prevent such outbreaks, especially given measles’ high contagion potential, where one infected individual can transmit the virus to up to 18 others in unvaccinated populations. What’s Driving The Rise Of GLP-1 Utilization In Children?

This article discusses the increasing use of GLP-1 receptor agonists among children and adolescents. This trend is driven by rising pediatric obesity rates, the approval of these medications for adolescents, and endorsements from medical organizations like the American Academy of Pediatrics. While these drugs offer potential benefits, including improved metabolic health and reduced risk of chronic diseases, concerns persist regarding long-term safety, appropriate dosing, and the psychological impact on young patients. The article emphasizes the need for careful consideration and monitoring when prescribing GLP-1 medications to the pediatric population. Full Access: Becker’s Hospital Review
