2023 North East Regional Urgent Care Conference - Speaker Agreement
This agreement outlines the terms and conditions between the North East Regional Urgent Care Association (CalUCA) and the Speaker.

Speaker agrees to:

  • attend and provide presentation(s) for this event. In the event Speaker is unable to attend, Speaker will provide a replacement or work with NERUCA to find one.
  • use templates provided by NERUCA (if applicable) for all presentation materials.
  • to acknowledge and source all photos, graphics, or other images used in presentation materials appropriately.
  • to reference all data appropriately.
  • to not willfully infringe on copyrights or plagiarize any content.
  • to finalize all presentation materials two weeks prior to the event.

Honoraria Terms and Conditions

  • Main Speakers receive complimentary registration plus $500 for contributing one session, $700 for two sessions, and $900 for three or more sessions.
  • NERUCA Board Member Speakers receive complimentary registration for contributing one session, and then $200 for two sessions, and $400 for three or more sessions.
  • Co-Speakers/Facilitators/Panelists receive $150 for contributing to one session, $350 for two sessions, and $550 for three or more sessions (does not include complimentary registration).

Note: The honoraria paid is to cover travel and hotel expenses, no need to send in expense receipts for reimbursement.

Exceptions: Vendor Speakers and Poster Presenters receive complimentary registration only.

  • Honoraria is based on contribution and will be determined at the completion of the event.
  • All honoraria payments will be processed via ACH (electronic funds transfer) within 30 days after the completion of the event.
  • Payment is dependent upon the receipt of a completed W-9 and a Direct Deposit Authorization form. In the event the forms are not received by November 21, 2023, no payment will be made.
